 Post subject: Shot in the dark - birth certificate?

My great-grandfather was born in Jonava, Lithuania in 1896. My family thinks - only thinks - this is something along the lines of a birth certificate. It is attached. We have been trying to piece together what this says, though we are completely stumped. We see that it references the town of Jonava, my great-grandfather (Joselis or Josel or Josif, etc.). It also appears to have other names Zelda Suntockies, Sevach Chait and others that we cannot read. It looks like it mentions Bagaslaviskis. There is a ton of other information on there as well that we cannot understand. Does anyone have the time and inclination to help? It would be MUCH appreciated. I can provide a higher res copy as well; this is limited to a small file. Thank you thank you thank you!!!
